Requests for BodyTalk should include the following information:

1. Name of Animal, Sex (Neutered/Spayed), Breed and approximate age.
2. Reason for Request (i.e., behavioral, health, physical problems, etc.).
3. Medications/Supplement, Treats and Food given to the Animal (dosage, how often, why prescribed, etc.).
4 Will you be able to observe the animal and provide follow-up emails/phone calls to monitor the animal's progress (no extra charge for followups)?
5. Any information about family members, animal's history, etc. that may help in focusing on "issues" that are affecting the animal.
